Our rehabilitation team creates an individualized program with our interdisciplinary team of nursing, social services, activities and dietary in conjunction with your physician. We work side by side with each resident and their loves ones to develop a care plan that meets your goals and abilities. Our goal is to be an integral role in helping you regain your health and reach your maximum functional ability before returning to the community.
